Job Opportunity: Home Health Registered Nurse

Location: Mount Angel, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM)
  • Schedule: 5 days per week
  • Patient Ratio: 1:1, with up to 6 patient visits per day (30 productivity points standard)
  • Start Date: ASAP
  • Weekend Work: May be required occasionally
  • Transportation: Must have own transportation; occasional travel radius up to 50+ miles
  • Attire: Any color scrubs
  • Charting System: EPIC (experience preferred)
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications

  • Valid RN license in Oregon
  • Minimum 1 year of home health nursing experience, with experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • BLS (Basic Life Support) certification issued by the American Heart Association
  • Experience in a Home Health setting
  • OASIS experience

  • First-Timers: Not accepted (must have prior home health experience)
  • On-Call: Not required
  • Floating: Not required
  • Block Schedule: Not accommodated
  • Locals: Candidates within 50 miles considered local and accepted
  • RTO Restrictions: Confirm with hiring manager

Compliance and Documentation

  • Background Check: Must be completed within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, motor vehicle record, and state-specific abuse/neglect searches as applicable
  • Drug Screen: 9-panel drug screen within 12 months of start (instant acceptable)
  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
  • Immunizations and Health Requirements:
    • TDAP vaccine within 10 years (declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Seasonal flu vaccine during flu season (declination accepted)
  • Respiratory Safety Clearance: Required annually for travel staff, including respirator medical clearance
  • Fit Mask Testing: Required annually for travel staff; onsite testing available if needed

Training and Competencies

  • Annual Regulatory Modules: Not required for home health at this facility
  • Epic Training: Preferred; candidates must complete assigned Epic modules within a week of orientation
  • Competency Exams: Medication and specialty-specific competency exams as applicable

References and Employment History

  • Minimum of 2 references covering at least 1 year of relevant employment within the last 3 years
  • Employment application covering at least the last 5 years of employment with explanations for any gaps or overlaps

Special Notes

  • Facility may call off traveler up to 1 shift per pay period (every two weeks)
  • COVID vaccination documentation preferred; declinations not required but unvaccinated staff must follow infection prevention protocols
  • Traveler must complete required background checks, health screenings, and certifications prior to start
  • Must comply with all state-specific licensing and background requirements for Oregon

This position is ideal for experienced home health RNs who are comfortable with travel assignments, have OASIS experience, and meet the vaccination and certification requirements.
