Description
Physical Therapist - Home Health
Location: Lexington, Kentucky
Contract Length: 13 weeks (approximately 3 months)
Shift: Days (typically 5 days/week, 8 hours/day)
Hours: 40 hours per week
Weekend: Rotating weekend shifts required (8 AM - 5 PM)
Holiday: Participation in holiday rotation required
Job Description
- Provide physical therapy services in a home health setting
- Manage approximately 6.5 patient visits per day/week, including start of care (SOC), resumption of care (ROC), recertifications, evaluations, and daily visits
- Common patient diagnoses include congestive heart failure (CHF), orthopedic conditions, and pneumonia
- No direct clinical involvement beyond patient care visits (profiles sent directly to manager for phone interview)
- Phone and tablet provided for work use
Required Qualifications
- Valid Physical Therapist license in Kentucky
- Minimum of 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- BLS (Basic Life Support) certification issued by the American Heart Association
- Experience with Homecare Homebase
Orientation & Support
- Two weeks of orientation with a preceptor, including a few visits with a work coach
- Access to clinical manager or educator support 24/7 for emergencies and 8 AM - 5 PM for routine questions
Work Environment & Attire
- Work primarily in patients’ homes
- Dress code: scrubs or business casual attire acceptable
Additional Notes
- Schedule is Monday through Friday, 8 AM to 5 PM, plus rotating weekends
- Emergency and clinical support available while in the field
