Description
Occupational Therapist - Acute Care Hospital (Travel/Contract)
Location: Lafayette, Indiana
Contract Length: 13 weeks
Schedule
- Monday through Friday, plus one weekend day per month
- Day shift: 8:00 AM – 4:30 PM (5 days/week, 8 hours/day)
Job Description
- Work as part of an inpatient acute care rehab team serving patients across all acuity levels, including ICU and progressive care units
- Patient diagnoses may include cardiac procedures (e.g., CABG), stroke, traumatic brain injury (TBI), joint replacement, fracture repairs, respiratory illnesses, dementia, multiple sclerosis (MS), Parkinson’s disease, and ambulation issues
- Responsibilities include evaluating patients, developing and following plans of care (POC) involving ADL training, therapeutic exercises, and activities to improve function
- Assess appropriateness of discharge disposition recommendations daily
- Charting in Cerner electronic health record system
- Dress code: Royal blue scrubs and black shoes
Required Qualifications
- Valid Occupational Therapist license in Indiana
- Minimum 1 year of Occupational Therapist experience within the last year
- Experience in an Acute Care setting
- Basic Life Support (BLS) certification (AHA or RQI accepted)
Preferred Qualifications
- Experience with Cerner electronic health record system
Required Documentation & Compliance
- Valid Driver’s License or Passport (clear copy)
- Background check within 90 days, including 7-year history and various national/statewide checks
- Education verification and official transcript or diploma (may be pending at start)
- Online verification of Indiana state license within 30 days before start
- Medication/pharmacology competency exam
- Unit competency exam
- Color blindness testing (Ishihara or equivalent, 100% score required)
- Physical exam within 12 months of start date
- Fit mask test within 12 months (not required for Physical Therapists)
- Immunizations and titers:
- Tuberculosis test within 1 year (PPD, QuantiFERON-TB Gold, or T-SPOT)
- Measles, Mumps, Rubella: Positive titer or 2 vaccines required (no declinations accepted)
- Varicella: Positive titer or 2 vaccines required (no declinations accepted)
- Hepatitis B series, titer, or signed declination required
- Tetanus/Diphtheria/Tdap strongly recommended
- Negative 10-panel drug screen within 12 months (including amphetamines, marijuana, cocaine, opiates, PCP)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/30 - 3/31)
- References: Minimum 2 from recent employment within 2 years, from charge, manager, supervisor, or above
- Current employment application with full work history and explanations for gaps over 30 days
- Skills checklist completed within 12 months and annually thereafter

- On-site fit testing and PAPR use require prior approval and documentation submission if applicable
