Description
Physical Therapist - Home Health (Travel/Contract)
Location: San Diego, California
Contract Length: 13 weeks
Job Description
- Provide physical therapy services in a home health setting covering Central/South San Diego territory.
- Home Health experience preferred; candidates without Home Health experience must have outpatient experience.
- OASIS experience required; candidates without OASIS experience may be considered if they have strong outpatient experience.
- Guaranteed hours based on completed visits: 25 visits equal 40 hours per week.
- Mileage reimbursement provided at IRS standard rates.
- COVID vaccination or approved exemption required.
- Day shift: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts).
Required Qualifications
- Valid California Physical Therapist license preferred; candidates with a pending CA license who have applied may be considered.
- Minimum years of experience: Not explicitly stated, but Home Health and OASIS experience required/preferred (implies relevant PT experience).
- Must have completed or be able to complete OASIS documentation.
Certifications & Compliance
- Basic Life Support (BLS) - AHA Provider or Instructor required.
- Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP), Electronic Fetal Monitoring (EFM) as applicable (verify based on assignment).
- Occupational Therapists are not required to have NBCOT certification; verify for PT if applicable.
- Medication/Pharmacology competency exam as applicable.
- Physical exam within 12 months of start date.
- Immunizations and health requirements:
- Tetanus/Diphtheria/Tdap vaccine within past 10 years (no declinations accepted).
- Negative TB test or chest X-ray within 1 year (with special rules if past positive).
- Measles, Mumps, Rubella (MMR) proof of immunity or vaccination for candidates born in 1957 or later (no declinations accepted).
- Hepatitis B titer or vaccine series (declination accepted if within 1 year).
- Varicella immunity or vaccination (no declinations accepted).
- Negative 9-panel drug screen within 1 year.
- Seasonal flu vaccination or documented declination required year-round.
- COVID vaccination or approved medical/religious exemption required.
Documentation & Background Checks
- Government-issued photo ID (Driver’s License or State ID).
- Valid passport copy required before start.
- I9 attestation required for clearance.
- Standard background check including 7-year history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG checks (all required annually).
- Photo for badge (passport style, shoulders up, front facing, plain background).
- Personal or auto insurance and auto registration required and must be kept current during assignment.
- Employment application and skills checklist required annually.
- Two professional references required.
- Training content and hand hygiene policy acknowledgments must be reviewed and signed prior to start.
Additional Notes
- Hours are guaranteed if 25 or more visits are completed weekly.
- COVID exemptions are permitted.
- Mileage reimbursement follows IRS standard rates.
- Candidates must have applied for California license if not currently licensed.
