Job Opportunity: Physical Therapist - Home Health

Location

• Portland, Oregon
• Travel/Contract position covering West Portland and surrounding areas (up to ~60 miles radius daily)

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapist)
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
• Productivity: May visit up to 6 patients per day; 30 productivity points standard
• Contract Length: 13 weeks
• Start Date: ASAP
• Weekend work: Not required
• On-call: Not required
• Floating between units: Not required
• Block schedules (e.g., 4x10s or 3x12s): Not accommodated
• Locals accepted (within 50-75 mile radius)
• Pending license accepted
• Guaranteed hours with possibility of being called off up to one shift every two weeks
• Own transportation required

Required Qualifications

• Valid Physical Therapist license in Oregon (pending licenses accepted)
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Basic Life Support (BLS) certification issued by American Heart Association or Heart & Stroke Foundation

Documentation & Compliance

• Employment application covering at least last 5 years of employment
• Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
• Education verification and licensure verification required
• Annual regulatory and specialty-specific training modules (Workramp) generally not required for home health at this facility
• Facility uses EPIC Remote Client and Rover app for charting
• Business casual attire; scrubs allowed but no specific color required; no jeans
• Free parking and mileage reimbursement at IRS standard rates

Additional Notes

• Facility does not accommodate block schedules or floating between units
• Traveler may be called off up to one shift per pay period
• Must comply with Oregon and Washington state-specific background and licensure requirements if applicable
• Must complete required health screenings and vaccinations prior to start
• Must have reliable transportation due to travel between patient homes
• No weekend or on-call requirements
