Physical Therapist - Home Health (Travel/Contract)
Location: Novato, California
Shift: Day, 5 days/week, 8 hours/day (8:00 AM - 5:00 PM)
Contract Length: 13 weeks
Job Summary
Provide physical therapy services in a home health setting. Collaborate with stakeholders to develop solutions and treatment protocols. Apply critical thinking, complex problem solving, and professional judgment independently. Maintain knowledge of professional practice and regulatory requirements. Manage multiple priorities and complete duties within expected timelines. Demonstrate strong communication, interpersonal, teamwork, and customer service skills. Ability to teach, develop training materials, and apply evidence-based practice to clinical care.
Required Qualifications
- Valid Physical Therapist license in California
- Basic Life Support (BLS) certification
- Valid driver's license
- Experience with Epic electronic health record system

Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender, education verification, motor vehicle report (for driving roles), FACIS Level 3, EPLS/GSA/SAM, HHS/OIG, OFAC sanctions, Medi-Cal Suspended and Ineligible Provider List
- Education verification (highest level achieved) required
- Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids due to CA law)
- Physical exam within 1 year of start
- Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or failed tests)
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- TB screening (negative QGold or TSPOT within 90 days or 2 PPDs) with annual renewal
- Measles (Rubeola), Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B series or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
- Respiratory safety clearance annually (not required for some roles)
- Drug testing and immunization documentation must be current and uploaded as required
Documentation and Verification
- Full 7 years of work history with no gaps, including city and state of employment
- Employment verification for up to 7 years, with explanations for gaps over 30 days
- Professional references: 2 required for clinical/allied/per diem acute care candidates
- Primary source license verification within 30 days of start
- Official transcripts/diplomas or attestation if verification unsuccessful
- Candidate must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and regulatory orientation prior to start
- TB questionnaire signed by candidate and healthcare provider within 90 days of start, renewed annually
Physical and Work Environment Requirements
- Must be able to work in a demanding environment, organize multiple priorities, and meet deadlines

Additional Notes
- Candidates with religious exemptions for facial hair or vaccines must complete specific exemption forms and obtain approval
- ICU & MS/Tele RNs at certain locations may have additional testing requirements (not applicable here)
- Some certifications (e.g., ACLS, PALS, CCRN) may be position-dependent and should be verified if applicable
