PT Home Health (Travel/Contract)

Location: Portland, Oregon (also covers Clark County, Washington and surrounding areas)
Contract Length: 13 weeks

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapy)
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
• Work Setting: Home Health
• Patient Load: 1:1 ratio, up to 6 patients per day (30 productivity points standard)
• Start Date: ASAP
• Schedule Notes:

  • No weekend work required
  • No on-call duties
  • No floating between units
  • No block scheduling or extended shifts (no 4x10s or 3x12s)
• Local Candidates: Accepted if within 50-75 mile radius, considered local

Required Qualifications

• Valid Physical Therapist license in Oregon
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Experience with OASIS documentation
• Basic Life Support (BLS) certification issued by the American Heart Association or Heart and Stroke Foundation

Facility & Work Environment

• Attire: Business casual preferred; scrubs allowed but no specific color required; no jeans
• Charting: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation
• Parking: Free
• Mileage Reimbursement: IRS standard rates

Additional Notes

• Facility may call off up to one shift per pay period (every two weeks)
• Confirm return-to-office (RTO) restrictions with hiring manager
• Annual regulatory and specialty-specific training modules are generally not required for Home Health at this facility
• Candidates must complete state-specific background and licensure attestations as applicable for Oregon and Washington
• Float policy acknowledgment may be required, but no floating is expected for this role
