RN Case Management
Job Overview
- Employment type: Travel
- Location: Dover, New Jersey
- Schedule: Monday–Friday, 8:00 AM–4:30 PM
- Hours per week: 40
- Contract length: 13 weeks
Role Summary
- Coordinates patient care across the continuum through collaboration with the interdisciplinary care transitions team.
- Supports assessment, care planning, plan implementation, evaluation, utilization review, discharge planning, and continuity of care.
Required Qualifications
- Active New Jersey RN license or multi-state RN license
- BLS certification through the American Heart Association
- 4 years of recent RN case management experience
- Experience in an LTAC or STACH setting
- Experience with MCG
- Experience with discharge planning across acute care settings
- Strong interdisciplinary team collaboration experience
