Patient Care / Workload
- Patient ratio: 1:1
- Daily volume: Up to 6 patients per day
- Productivity standard: 30 points
- Travel radius: Occasional maximum radius of roughly 60 miles per day
Required Qualifications
- Valid Physical Therapist license in Oregon
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS
- Experience in a Home Health setting
- Experience with OASIS
- Transportation: Own transportation required
Additional Notes
- First-time travelers accepted
- Local candidates accepted within a 50–75 mile radius
- Facility may call off up to one shift per pay period
- Primary series COVID vaccination plus one booster required
- Facility cannot accommodate 4x10 or 3x12 schedules
