Job Overview

- Location: Mount Vernon, Ohio

- Shift: Day, 5x8 hours
- Hours: 7:30 AM–4:00 PM

- Caseload: Full caseload with a variety of diagnoses

Required Qualifications
- Valid Physical Therapist license in Ohio
- BLS issued by American Heart Association or American Red Cross
Required Documents and Health Requirements
- Government-issued photo ID
- Proof of E-Verify or attestation
- Background check with 7-year criminal history review
- FBI and BCI fingerprinting within 12 months of start
- Primary source license verification
- Physical exam within 12 months of start
- Negative TB test or chest X-ray within 1 year of start
- Tdap documentation
- Measles, mumps, rubella, and varicella immunity documentation
- Hepatitis B documentation
- 9-panel drug screen within the last 12 months
- Flu vaccination or approved exemption during flu season
