Required Qualifications

Valid RN license in Oregon
Home Health (1 year of experience)
Home Health experience within the last year
One completed travel assignment
BLS issued by American Heart Association or Canadian Heart & Stroke Foundation
Associate of Science in Nursing / Associate Degree in Nursing
Experience in a Home Health setting
Experience with OASIS

Key Responsibilities

Home health visits in the West Salem/Keizer area and surrounding areas, with potential coverage for Woodburn
May visit up to 6 patients per day
Expected to have personal transportation
Occasional travel radius may exceed 50 miles

Additional Notes

First-time travelers are not accepted
Local candidates are accepted within a 50-mile radius
Primary series COVID vaccination plus one booster is required
Epic experience is preferred
Guaranteed hours may be reduced by up to 1 shift per pay period
Free parking is available
