Schedule

• Shifts: 3 or 4 shifts per week
• Shift hours: 12-hour day shifts (7:00 AM – 7:30 PM)
• Guaranteed hours: 36 hours/week (up to 48 hours available; overtime only after 48 hours)
• Weekend requirement: Every other weekend
• On-call: No
• Floating: Yes, within scope (no floating to sister facilities)
• Block scheduling: Not available

Requirements/Qualifications

• Certified Respiratory Therapist with NICU experience (minimum 1 year, with experience within the last year)
• Pediatric Respiratory Therapist experience
• Current Basic Life Support (BLS) certification
• Current Pediatric Advanced Life Support (PALS) certification

Time-Off Policy

• Maximum 3 shifts off for 13-week contracts
• Maximum 8 shifts off for 26-week contracts
• Taking 1 full week off requires adding an additional week to the contract
• No time off approved during these blackout periods:

  • August 31 – September 6
  • November 23 – November 29
  • December 21 – January 3

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License/State ID)
• Background checks covering all locations lived/worked/schooled in past 7 years, including SSN trace, sex offender registry, education verification, and others
• Education verification and transcript/diploma (transcript accepted if education verification pending)
• Professional license copy and primary source verification
• Professional references: 2 within past 2 years (preferably supervisors/managers), cannot be pending
• Employment application and skills checklist

Required Certifications & Competencies (must be valid and renewed prior to start)

• ACLS (Advanced Cardiac Life Support) if applicable to unit/specialty
• BLS (Basic Life Support)
• PALS (Pediatric Advanced Life Support)
• Neonatal Resuscitation Program (NRP) if applicable
• Chemotherapy certification if applicable
• Medication test/pharmacology competency exam
• Color blindness testing (can be completed online or as part of physical)
• Competency exam
• Orientation safety manual acknowledgement
• Pass/fail medication calculation exam (not required for per diem)

Health Requirements

• Physical exam within 30 days of start (must include latex allergy information)
• Fit test for N95 or PAPR respirators within 1 year of start, updated annually
• Immunizations and titers:

  • Tdap vaccine within 10 years (TD and DTap not accepted)
  • Negative TB test (PPD, QGold, or TSPOT) within 60 days of start; chest x-ray if past positive or new converter
  • Measles, Mumps, Rubella: 2 MMR vaccines or positive quantitative titer with lab values and reference ranges
  • Hepatitis B series and quantitative surface antibody titer with lab values; declination form accepted if declining vaccine
  • Varicella: 2 vaccines or positive quantitative titer with lab values (LabCorp exceptions apply)
• 10-panel drug screen within 30 days of start (must include marijuana)
• Seasonal flu vaccination or approved medical/religious exemption form required during flu season
• COVID-19 vaccine or declination required for tracking purposes
• Latex questionnaire if latex allergy info not included on physical exam
• TB questionnaire required for all candidates

Additional Facility-Specific Requirements

• Photo for badge (must meet strict guidelines; no altered photos or government ID photos)
• Completion of required training modules prior to start (including COVID-19 vaccine training and other facility-specific modules)
• Emergency contact information
• Hepatitis B declination form if applicable
• Flu exemption forms if applicable (medical or religious)
• TB exposure/PPD conversion record if applicable
• Medication study guide provided for nursing staff to prepare for orientation testing
