- Shift: 8-hour days
- Schedule: 5x8, 8:00–16:30
- First-time travelers accepted
- Local candidates accepted
- No weekend requirement

Required Qualifications
- Valid Physical Therapist license in Oregon
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS issued by American Heart Association or Heart and Stroke Foundation
- Experience in a Home Health setting
- Experience with OASIS
Schedule Notes
- Facility cannot accommodate 4x10 or 3x12 schedules
- May visit up to 6 patients per day
- Standard productivity: 30 points
- Occasional daily travel radius may be up to about 60 miles, with total drive time of 60–90 minutes
Work Environment
- Home health
- Business casual attire; scrubs allowed but not required
Additional Notes
- Local candidates are considered within a 50–75 mile radius
- RTO restrictions should be confirmed with the hiring manager
- Facility may call off up to one shift per pay period
- Mileage reimbursement is provided at the IRS standard rate
- Free parking is available
