Required Qualifications
- Valid Physical Therapist license in Texas
- Physical Therapist (2 years of experience)
- Physical Therapist experience within the last year
- Experience in a Long Term Care setting
Role Summary
- Performs patient evaluations and develops individualized treatment plans
- Reassesses patients throughout treatment and manages discharge planning
- Supervises ancillary staff and students as allowed by state law and department policy
- Documents care accurately and communicates with patients, families, caregivers, and the care team
- Provides patient and caregiver education, participates in quality improvement, and follows safety standards
