- In-home, patient-centered care

- Home-based practice
Required Qualifications
- Valid Physical Therapist license in Colorado
- BLS
- Driver's License
Preferred Experience
- Home health experience
- Familiarity with Medicare documentation and home health regulations
- Experience working with geriatric and medically complex populations
Key Responsibilities
- Perform physical therapy visits, including evaluations, start of care, reassessments, recertifications, and discharge visits
- Develop and implement individualized plans of care
- Provide gait training, balance training, therapeutic exercise, mobility training, and fall prevention
- Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
- Communicate patient progress or changes in condition to the interdisciplinary care team
- Collaborate in the clinical oversight of PTAs when applicable and permitted under Colorado law and agency policy
- Complete documentation accurately and on time
- Participate in care coordination related to patient needs, visit completion, and regulatory requirements
