    Physical Therapist

    • Monday–Friday, 8:00 AM–5:00 PM

    Contract Length

    • 13 weeks

    Work Setting

    • In-home, patient-centered care
    • One-on-one visits
    • Home-based practice

    Required Qualifications

    • Valid Physical Therapist license in Colorado
    • BLS
    • Driver's License

    Preferred Experience

    • Home health experience
    • Familiarity with Medicare documentation and home health regulations
    • Experience working with geriatric and medically complex populations

    Key Responsibilities

    • Perform physical therapy visits, including evaluations, start of care, reassessments, recertifications, and discharge visits
    • Develop and implement individualized plans of care
    • Provide gait training, balance training, therapeutic exercise, mobility training, and fall prevention
    • Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
    • Communicate patient progress or changes in condition to the interdisciplinary care team
    • Collaborate in the clinical oversight of PTAs when applicable and permitted under Colorado law and agency policy
    • Complete documentation accurately and on time
    • Participate in care coordination related to patient needs, visit completion, and regulatory requirements
