RN Visit Nurse — Home Health

Required Qualifications

Associate’s degree
Current Washington State Registered Nurse license
Current BLS certification
Valid Washington driver’s license
Proof of insurance on automobile used for work
Reliable automobile transportation to travel to patient residences
Driving record in compliance with employer standards
Cell phone with activated voicemail
Computer internet connectivity

Experience

Minimum of 1 year recent experience in home health, acute care, or long-term care

Role Summary

Provides nursing visits in patients’ homes as part of the interdisciplinary care team
Supports continuity of care by carrying out delegated visits and communicating patient progress and physician orders to the case manager RN
