RN Case Manager
Job Overview
- Location: Chillicothe, Ohio
- Employment type: Contract
- Schedule: Days, Monday–Friday, 7:30 a.m.–4:00 p.m.
- Hours per week: 40
- Contract length: 13 weeks
- Caseload: About 20 patients per day
Role Summary
- Assess discharge needs, social determinants of health, and discharge planning/referrals for acute medical and surgical patients.
Required Qualifications
- Valid RN license in Ohio
- Associate of Science in Nursing / Associate Degree in Nursing
- Acute Care Case Management (3 years of experience)
- Acute Care Case Management experience within the last year
- BLS/BCLS
- Must reside outside a 60-mile radius
