Job Overview

• Travel/contract role in Mount Angel, Oregon, serving Mount Angel and surrounding areas in the northern territory.
• Start date: ASAP
• Contract length: 13 weeks
• Shift: 8-hour days
• Schedule note: This facility cannot accommodate 4x10s or 3x12s.
• Patient ratio: 1:1
• May visit up to 6 patients per day
• Standard productivity: 30 points
• Home health setting

Required Qualifications

• Valid Physical Therapist license in Oregon
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association or Canadian Heart & Stroke Foundation
• Experience in a Home Health setting
• Pending license accepted
• First-time travelers accepted
• Own transportation required

Schedule and Work Expectations

• Weekend requirement: No
• On-call requirement: No
• Floating between like units: No
• Block scheduling: Not accommodated
• Local candidates accepted within a 50–75 mile radius
• May be called off up to one shift per pay period

Additional Considerations

• Primary series COVID vaccination plus one booster required
• Occasional travel radius may be up to about 60 miles per day, with 60–90 minutes total drive time
• RTO restrictions: confirm with hiring manager
