Job Overview
- Contract role in Petaluma, California
- 40 hours per week
- Skilled nursing facility setting
Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS
- Bachelor's, Master's, or Doctoral degree in Physical Therapy
Preferred Experience
- Experience working in a skilled nursing facility or similar healthcare setting
Key Responsibilities
- Evaluate patients’ physical abilities and develop individualized treatment plans
- Provide physical therapy interventions to improve strength, mobility, and functional abilities
- Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
- Collaborate with the interdisciplinary care team
- Monitor progress and adjust treatment plans as needed
- Document assessments, progress notes, and treatment plans accurately and on time
