Job Overview

  • Travel/contract physical therapy role in home health
  • Day shift, 5x8 hours
  • 40 hours per week
  • 13-week contract
  • Field-based coverage across multiple locations on the island
  • Local candidates accepted

Required Qualifications

  • Valid Physical Therapist license in Hawaii
  • Physical Therapist (1 year of experience)
  • Physical Therapist experience within the last year
  • BLS - Basic Life Support issued by American Heart Association
  • Experience in a(n) Home Health setting
  • Driver's License
  • Experience with OASIS

  • Government-issued photo ID required
  • Valid passport required before start
  • Passport-style photo required before start
  • Personal or auto insurance required
  • Auto registration required
  • Physical exam within 12 months of start required
  • Tdap within the past 10 years required
  • Negative TB test or chest X-ray within 1 year of start required
  • Measles, mumps, and rubella immunity documentation required for candidates born in 1957 or later
  • Hepatitis B documentation required
  • Varicella documentation required
  • Negative 9-panel drug screen within 1 year of start required
  • Seasonal flu vaccination or documentation required year-round
  • COVID vaccination or approved medical/religious exemption required
  • Background check required
  • EPLS/GSA/SAM check required
  • HHS/OIG check required
  • Employment application required
  • Skills checklist required annually
  • Training and hand hygiene acknowledgements required before start

Work Details

  • Mileage between patients reimbursed at IRS standard rate
  • Hours are based on completed visits
  • 25 completed visits equals 40 hours
  • Hours are guaranteed when 25 or more visits are completed
  • COVID exemptions permitted
