PT Home Health

Job Overview

  • Location: Seaside, Oregon
  • Employment type: Travel/Contract
  • Start date: ASAP
  • Shift: Day shift, 8 hours
  • Schedule note: Facility cannot accommodate 4x10s or 3x12s
  • Setting: Home health
  • Contract length: 13 weeks

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • 1 year of Physical Therapist experience
  • Physical Therapist experience within the last year
  • BLS - Basic Life Support issued by American Heart Association or Heart and Stroke Foundation
  • Experience in a Home Health setting

Schedule and Work Expectations

  • Weekend work not required
  • On-call not required
  • No floating between like units
  • Block scheduling not available
  • May visit up to 6 patients per day
  • Standard productivity: 30 points
  • Traveler must have their own transportation
  • Occasional travel radius of roughly 60 miles per day

Location and Candidate Preferences

  • Local candidates accepted within a 50–75 mile radius

Compliance and Health Requirements

  • Primary series COVID vaccination plus one booster
  • Physical exam within 1 year of start
  • Fit mask required
  • Tdap within 10 years
  • Negative TB test or chest X-ray
  • Measles, mumps, rubella, varicella, and hepatitis B documentation required
  • Drug screen required
  • Flu vaccination or declination required during flu season
  • Respirator clearance required

  • Free parking
  • Mileage reimbursed at IRS standard rate
  • Business casual attire; scrubs allowed but not required
  • Charting uses EPIC Remote Client for Home Health, Rover, and Oasis
