Clinical Care Registered Nurse

Schedule
- Days, Monday through Friday, 8:00 AM to 4:30 PM
- 40 hours per week
Required Qualifications
- Valid RN license in Michigan
- Associate of Science in Nursing / Associate Degree in Nursing
- Clinic experience within the last year
- Minimum of one year of experience working with the frail, elderly, or long-term care population
- BLS issued by American Heart Association
- Driver's License
- Experience working with Geriatric patients
- Geriatrics experience
- Wound Care experience
- Home Health experience
Key Responsibilities
- Provide direct nursing care to participants
- Triaging emergent situations
- Monitor acute and chronic medical conditions in coordination with primary care providers
- Use care plans independently to provide clinic and home-based direct care
- Administer treatments, procedures, and medications as prescribed
- Make regular home visits, including wound care, lab draws, nurse assessments, and other nursing tasks

Work Environment
- Works independently and collaborates with licensed and non-licensed personnel
- Visible presence in the center with frequent interruptions
- May work beyond normal hours, weekends, and holidays as needed
