Home Health Physical Therapist
Overview
- Specialty: Home Health PT
- Location: Stockton, California
- Contract length: 13 weeks
- Shift: Days
- Hours: 8-hour shifts, 40 hours per week
- Schedule: Monday–Friday, 8:00am–5:00pm
- Patient volume: 4–5 patients per 8-hour day
- Call: No call required
- Weekends: Not required; optional if desired
Required Qualifications
- BLS certification
- California PT license
- Valid driver's license
- Car insurance
- 1 year of PT experience
- Prior home health experience
Preferred Qualifications
- Home health experience is preferred
Responsibilities
- Provide regular PT care
- Complete documentation within 1 day of the patient visit
Additional Notes
- All required modules must be completed prior to arrival
- Mileage is billable per GSA rate
- Patients are all ages with multiple comorbidities
- EMR: HCHB (HomeCare HomeBase)
