Physical Therapist (PT) — Home Health

Overview

• Temp role in Pittsfield, Massachusetts
• Days shift, 8 hours per day
• 13-week contract
• Home health assignment

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (2 years of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association
• Experience in an Acute Care setting
• Experience in a Home Health setting
• Driver's License
• 2 or more years of acute nursing care experience
• 2 or more years of home care experience
• Experience with OASIS

Preferred Qualifications

• Acute care experience

Assignment Notes

• Traveler must live more than 50 miles from the facility
• Traveler must have their own vehicle
• Requested time off must be included and verified at upload; no more than 5 days of RTO will be accepted
• Scheduling requests must be made at the time of submission
• Must be able to pass a fit test using an N95 mask
• Mileage reimbursement applies only to the office and/or drop/satellite office, not to home-to-housing travel
• Voicemail must be set up on the phone
• Perm address must be included in the profile
• Client prefers a 45-minute or less commute from the office
