Required Qualifications

• Doctoral degree in Physical Therapy, unless licensed prior to 2016 and grandfathered under prior licensure requirements
• Illinois license in Physical Therapy, or graduation from a professional program with Authorization to Test Letter, scheduled to test within 60 days of eligibility, and no failed licensure exam
• CPR certification within 90 days of hire
• Previous experience as a Physical Therapist
• Valid state driver’s license
• Active Illinois license or confirmation that the application process has been initiated
• Negative drug screen for all panels
• BLS Certification

• Ability to evaluate patients’ physical and functional status, establish therapy goals, formulate treatment plans, observe patients, document status, and assist techs and assistants in resolving problems
• Knowledge and skills necessary to provide patient care appropriate to the population served
• Excellent computer skills and ability to process detailed information accurately
