- Travel/Contract role in San Mateo, California
- Day shift: 5x8 hours, 8:00–17:00
- 13-week contract
- Locals accepted within a 50-mile travel requirement; local candidates are paid at a lower bill rate
- Home health assignment
Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist (2 years of experience)
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
Required Documents / Compliance
- Personal or auto insurance required for all candidates working at this facility
- Background check required, including a 7-year background check within 6 months of start
- Motor Vehicle Report required for all candidates working at this facility
- Sex offender search required within 90 days of start
- FACIS Level 3 search required within 2 weeks of start and monthly after start
- California Medi-Cal search required if FACIS III cannot be run monthly
- EPLS/GSA/SAM required
- HHS/OIG required
- State license verification required within 30 days of start
- Physical exam required within 1 year of start
- Fit test required within 1 year of start and renewed annually
- TB screening questionnaire required within 90 days of start and renewed annually
- Negative TB test or chest x-ray required within 90 days of start
- Drug screen required within 6 months of start
- Flu vaccination/documentation required during flu season
- Color blind exam required before start
- Employment application must include full 7-year work history with no gaps
- Work history verification required for up to 7 years of employment history
- Unit-specific skills checklist required
- Candidate must review and sign job description, standards of business conduct, general regulatory orientation, vendor training, code of conduct, and overpayment refund policy before start
