Home Health RN
Job Overview
- Travel/contract role in Roseburg, Oregon
- Home health setting
- Day shift, 5x8 schedule
- 13-week assignment
- 40 hours per week
Key Responsibilities
- Provides and coordinates home nursing care based on physician orders, agency policies, and the nursing process
- Supervises and evaluates home health aides and LPN/LVNs
- Typical caseload is 5–8 patients per day
- Home visits are required, with periodic office meetings
- Territory coverage includes Roseburg and surrounding areas
Required Qualifications
- Valid RN license in Oregon
- Associate of Science in Nursing / Associate Degree in Nursing
- Home Health experience within the last year
- 1 year of Home Health experience
- Hospice experience
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Driver's License
- Experience with Homecare Homebase
- Experience with OASIS
Schedule and Work Expectations
- Weekend schedule varies by location and profession
- Potential for rotating on-call coverage
- Standard schedule is typically Monday through Friday, 8:00 AM to 5:00 PM
- Some positions may use a Baylor shift schedule of Friday through Monday, 4x10s
Charting and Documentation
- Must be proficient with HomeCare HomeBase and have used it within the last 6 months
- OASIS proficiency is required for RNs
Additional Notes
- Local candidates are accepted at the same rate
- Guaranteed hours are not provided
- Scrubs are required
- Mileage is reimbursed at the IRS standard rate
