PT - Therapy

Overview

Required Qualifications

• Valid Physical Therapist license in Illinois
• Physical Therapist experience within the last year
• 1 year of Physical Therapist experience
• Bachelor's degree in physical therapy
• BLS issued by American Heart Association

Responsibilities

• Performs patient transfers with assistance as needed from bed, chair, and floor, if applicable
• Uses safe body mechanics to reduce risk of injury
• Designs and adjusts the plan of care based on patient response and available evidence
• Educates patients, family, caregivers, students, and other health care providers
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care
• Performs examinations using reliable and valid methods relevant to the chief complaint, screening results, and history
• Produces documentation in accordance with the Clinical Process Policy
• Screens patients to determine the effectiveness of and need for therapy services, if appropriate
• Uses support personnel according to legal standards and ethical guidelines and monitors their care
• Completes annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements, if applicable
• Evaluates impairments related to orthopedic and neurologic conditions
• Establishes a plan of care based on functional deficits and patient goals
• Completes objective testing for range of motion, strength, and daily functional tasks
• Completes daily documentation and charging in therapy EMR
• Applies tape for stabilization, facilitation, or inhibition of movement
• Completes therapeutic modalities as appropriate, including ultrasound, interferential, and iontophoresis
• Communicates with the referring provider on patient progress and treatment recommendations
