Description
Hospice
Overview
- Travel/Contract role in Cartersville, Georgia.
- 8-week assignment.
- Day shift: Monday–Friday, 8:00–17:00.
- Hospice setting.
- Territory includes Bartow and Paulding County.
- Reports to the Cartersville location.
- No guaranteed hours.
Schedule and coverage
- Weekend rotation: backup one time a month.
- On-call: backup 2 times a week.
- May float to a nearby clinic for additional hours if patient census is low.
Responsibilities
- Provide home visits and attend periodic office meetings.
- Case types may include:
- Start of Care
- Wound Care
- Follow-up visits
- Other adult home health or hospice services
- Average daily caseload: 5 to 8 patients.
- Weekly productivity expectation: 30 points.
- Must be willing to travel up to 75 miles from the home clinic.
- Travel between patient homes is reimbursable.
Required Qualifications
- Valid RN license in Georgia.
- Associate of Science in Nursing / Associate Degree in Nursing.
- Hospice (1 year of experience).
- Hospice experience within the last year.
- One completed travel assignment.
- BLS.
- Experience in a Home Health or Home Health Hospice setting.
- Driver's License.
- Experience with Homecare Homebase.
- Experience with OASIS.
Preferred Qualifications
- Prior HCHB experience outside the six-month window may be considered if retraining is completed during compliance.
Additional Notes
- Locals are accepted at the same rate.
- Dress code: scrubs.
- Mileage is reimbursed at the IRS standard rate.
- Travel to the first patient and from the last patient is not billable.
