Required Qualifications

• Valid Occupational Therapist license in Massachusetts
• Occupational Therapist (2 years of experience)
• Occupational Therapist experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Bachelor's degree or higher
• Experience in a(n) Inpatient setting

Required Documents / Profile Items

• Work history
• Skills checklist and occupational health documentation, current to 1 year
• 1 supervisory signed reference from within the past two years of employment
• Traveler’s permanent address
• Traveler must have their own vehicle

Scheduling / Availability Requirements

• Requested time off must be noted and verified at upload
• No more than 5 days of requested time off will be accepted
• Travelers may not take all holidays off
• Must work two holiday blocks if scheduled/needed:

  • Thanksgiving Day, Friday after, and weekend after
  • Christmas Eve through Christmas Day and weekend after
  • New Year’s Eve through New Year’s Day and weekend after

Assignment Notes

• Do not submit a traveler who wants to work in conjunction with another perm placement or travel assignment
• 8 hours of non-billable orientation
