HumanEdge Health is seeking a dedicated and experienced Long-Term Acute Care (LTAC) RN for a night shift travel assignment in St. Louis, Missouri. This is a great opportunity for a skilled nurse ready to make a difference in a fast-paced clinical environment.

Assignment Details:

• Location: St. Louis, MO

• Shift: Nights (3x12s)

• Duration: 13 weeks

• Start Date: ASAP

Key Responsibilities:

• Deliver high-quality nursing care in accordance with the nursing care plan and physician direction.

• Supervise LPNs, CNAs, and new staff/orientees.

• Perform comprehensive nursing assessments within 8 hours of admission to guide individualized plans of care.

• Participate in interdisciplinary team meetings and communicate patient updates in a timely manner.

• Lead patient and family education with a focus on rehabilitation and discharge planning.

• Initiate and coordinate discharge plans from the first day of admission.

• Ensure continuity of care by effectively communicating patient status changes to providers and fellow caregivers.

• Delegate responsibilities appropriately while providing guidance and mentorship to support staff.

• Contribute to a safe environment by following safety protocols including those for infection prevention, fall risk, safe patient handling, and proper materials use.

• Accurately document interventions, assessments, and patient responses according to hospital policy.

• Actively participate in quality improvement and team-based care initiatives.

Requirements:

• Active RN license (Missouri or compact)

• BLS (Basic Life Support) certification

• Minimum of 1 year of recent acute or LTAC nursing experience preferred

• Strong clinical skills, critical thinking, and communication abilities
