HumanEdge Health is seeking an experienced Travel Physical Therapist for a contract opportunity in a Long-Term Acute Care (LTAC) setting. The PT will assess and treat patients with physical impairments or disabilities, create individualized care plans, and work collaboratively with a multidisciplinary team to support optimal patient recovery.

Key Responsibilities:

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans.

• Utilize clinical reasoning to implement and modify interventions focused on achieving discharge goals.

• Manage caseload effectively, including individual and group therapy sessions, while ensuring continuity of care.

• Collaborate with the interdisciplinary team to address discharge barriers and plan equipment needs, family training, and continuing care services.

• Provide timely and accurate documentation, including evaluations, daily notes, progress reports, and discharge summaries.

• Ensure treatments are provided under valid physician orders and within authorized insurance limits.

• Participate in student mentoring programs and provide appropriate supervision and evaluations.

• Pursue professional development through continuing education, self-study, and participation in professional activities and organizations.

• Maintain professional licensure and fulfill state continuing education requirements.

• Contribute to program development and actively engage in patient and professional advocacy efforts.

Qualifications:

• Active Physical Therapist license in Mississippi (or compact state eligibility)

• Graduate of an accredited Physical Therapy program

• Minimum 1 year of recent experience in an LTAC, acute, or inpatient rehab setting preferred

• BLS certification required

• Strong clinical, organizational, and communication skills
