Licensed Practical Nurse (LPN) – Full-Time & Per Diem
Brooklyn, NY | Skilled Nursing Facility | Days & Evenings with Weekend Coverage
We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our team at a well-established Skilled Nursing community in Brooklyn. This is an excellent opportunity for nurses who value providing quality care in a supportive and resident-centered environment.

1 Full-Time LPN – Day & Evening Shifts, including weekend coverage
- 1 Per Diem LPN – Flexible availability, days & evenings, weekend coverage required
Job Duties:

Provide direct nursing care and oversee resident care plans

Administer medications and treatments as prescribed

Monitor residents’ health and report changes to the RN or physician

Maintain accurate documentation and compliance with regulations

Collaborate with interdisciplinary team members to ensure the highest quality of care
Education:
NYS LPN
Certifications:
BLS
Experience & Skills:
-
Current NY Licensed Practical Nurse (LPN) license

Previous experience in assisted living, long-term care, or similar setting preferred

Strong communication and teamwork skills
