Occupational Therapist (OT) – Full-Time | Brooklyn, NY

Daytime Schedule | Community-Based Care | Geriatric Focus

What You’ll Do

  • Evaluate participants and develop individualized occupational therapy treatment plans

  • Provide hands-on therapy in clinic settings and participant homes

  • Collaborate closely with providers, IDT members, and specialists

  • Document evaluations, progress notes, discharge summaries, and care plans in the EMR

  • Recommend adaptive equipment and support safe discharges

  • Participate in care plan meetings and performance improvement initiatives

  • Provide clinical guidance to COTAs and support OT students as needed

  • Maintain compliance with all clinical, safety, and regulatory standards

What We’re Looking For

Required Qualifications

  • Bachelor’s degree from an accredited Occupational Therapy program

  • Active New York State Occupational Therapist license

  • Ability to travel locally within Brooklyn to participant homes and sites

  • Strong communication, documentation, and clinical judgment skills

Preferred Experience

  • At least 1 year of OT experience, ideally with geriatric or frail populations

  • Familiarity with community-based care or PACE-style programs

  • Experience using EMRs (Care Compass a plus)

  • Bilingual skills are a plus, depending on participant population

Physical & Professional Readiness

  • Ability to lift up to 50 lbs and assist participants safely

  • Comfortable with standing, walking, bending, and in-home visits

  • Up-to-date immunizations and medical clearance required

  • Commitment to ethical practice and ongoing professional development
