Location: Naples, FL
Duration: 13 Weeks
Start Date: 01/12/2026
Shift: Days
Schedule: 5x8s, 8:00 AM – 4:30 PM (rotating weekends)
Position Overview:
HumanEdge Health is seeking an experienced Travel RN for a Care Management role in a fast-paced hospital setting in Naples, Florida. This assignment focuses on care coordination and discharge planning across the continuum of care.
Responsibilities:

Collaborate with the interdisciplinary healthcare team to support effective resource management

Determine medical necessity for admissions and continued stays
-
Identify inappropriate or untimely services and opportunities to improve patient care
-
Coordinate discharge planning for patients of all ages to all levels of post-acute care

Facilitate timely, appropriate hospital discharges to reduce length of stay

Monitor cost-effective care delivery across the continuum

Maintain accurate and compliant documentation per regulatory requirements
Requirements:

Active Florida RN license or multistate compact license
-
BLS certification
-
Minimum of 2 years of hospital Care Management experience

Cerner EMR experience required

Ability to manage a high-volume caseload (approximately 1:25 ratio)
