NOW HIRING: Field RN Case Manager – Home Visits (Bronx, NY)
Full-Time | Community-Based | Travel Required

A respected healthcare organization is seeking two Field RN Case Managers to provide hands-on, community-based nursing care throughout the Bronx. This role is ideal for an RN who enjoys autonomy, direct patient interaction, and the ability to make a meaningful impact through home-based care.

The Field RN Case Manager will conduct routine home visits to perform wound care, administer medications, and complete health assessments as needed. Strong time management and independent decision-making skills are essential.

Position Overview:

• Full-time | 100% field-based

• Location: Bronx

• Schedule: Monday–Friday, 8:30 AM–5:30 PM (1-hour lunch)

• Weekend Rotation: 1–2 weekends per month

• Visit Expectation: Approx. 25 home visits per week

• Contract Length: Possible temp-to-perm

• Positions Available: 2

Must-Haves

• Active New York State Registered Nurse (RN) license

• Reliable personal vehicle and valid driver’s license

• Strong clinical assessment capabilities

• Experience with wound care and medication administration

• Ability to work independently in the field

• Excellent communication and documentation skills

• Comfort traveling throughout the Bronx

Nice to Have

• Home care or field nursing experience

• Bilingual Spanish or other languages

• Experience with diverse or underserved populations

• Strong route planning and organizational skills

Key Responsibilities

• Conduct routine home visits for assigned caseload

• Complete wound care, medication administration, and general assessments

• Maintain accurate documentation in accordance with policy

• Communicate changes in patient condition to appropriate care teams

• Coordinate care plans and follow-up needs

• Participate in rotating weekend schedule with compensatory weekdays off
