<h3>Position Overview</h3>

HumanEdge Health is seeking a Travel Physical Therapist Assistant (PTA) for a home care assignment in Mansfield, CT. This role provides outpatient and home-based physical therapy services, supporting patients in maintaining mobility, strength, and independence.


<h3>Responsibilities</h3>

<ul>
	<li>
	Provide physical therapy interventions in patients’ homes according to the plan of care
	
	</li><li>
	Assist with exercises, mobility training, and functional activities
	
	</li><li>
	Monitor patient progress and report outcomes to the supervising PT
	
	</li><li>
	Educate patients and families on exercises, safety, and home modifications
	
	</li><li>
	Maintain accurate documentation in compliance with agency and regulatory standards
	
	</li><li>
	Collaborate with interdisciplinary teams as needed
	
</li></ul>


<h3>Qualifications</h3>

<ul>
	<li>
	Current Connecticut PTA License required prior to start
	
	</li><li>
	Minimum of 1 year home care experience preferred
	
	</li><li>
	Strong interpersonal and communication skills
	
	</li><li>
	Ability to work independently in home care settings
	
	</li><li>
	Reliable, professional, and compassionate approach to patient care
	
</li></ul>
