HumanEdge Health is seeking an experienced Travel Occupational Therapist (OT) for a 13-week contract assignment in McMinnville, Oregon. This opportunity is based in a 24-hour nursing home setting with rehab services, offering a collaborative environment focused on functional independence, safety, and patient-centered care.

Schedule: Monday–Friday

Hours: 40 hours per week (5x8s)

Setting: Skilled Nursing / Rehab
Responsibilities:
-
Evaluate and treat patients with functional impairments impacting activities of daily living
-
Develop individualized treatment plans to improve independence, mobility, and safety
-
Collaborate with interdisciplinary teams including PT, SLP, nursing, and case management
-
Document evaluations, progress notes, and care plan updates in accordance with facility standards
-
Support rehabilitation goals to enhance quality of life and functional outcomes
Requirements:

Active Oregon OT License (or ability to obtain prior to start)

Minimum 1 year of OT experience; nursing home or rehab experience preferred

Current BLS Certification (AHA)

Strong clinical judgment, communication, and documentation skills
