HumanEdge Health is seeking a qualified Travel LMSW for a long-term assignment supporting the Clinical Care Management team in Valhalla, NY. This is a full-time, weekday position offering consistent hours in a clinical care environment.

Assignment Details 
Location: Valhalla, NY 
Start Date: ASAP 
Duration: 30 weeks 
Schedule: Monday–Friday 
Hours: 8:00 AM – 4:30 PM 
Guaranteed Hours: 40 hours per week 
Department: Clinical Care Management

Role Overview

<ul>
	<li>
	Provide clinical social work services within a care management setting
	
	</li><li>
	Coordinate care planning and support patient discharge and transitions of care
	
	</li><li>
	Collaborate with interdisciplinary healthcare teams
	
	</li><li>
	Assess psychosocial needs and connect patients with appropriate resources
	
	</li><li>
	Maintain accurate documentation in accordance with regulatory standards
	
</li></ul>

Requirements

<ul>
	<li>
	Master’s degree in Social Work (MSW) from a CSWE-accredited program
	
	</li><li>
	Active New York State LMSW or LCSW license and current registration required
	
	</li><li>
	Strong clinical assessment, communication, and documentation skills
	
	</li><li>
	Healthcare or care management experience preferred
	
</li></ul>
