Job Opportunity: Social Work Care Coordinator (Discharge Planning)

Job Description

• Work at a large Level 1 Trauma center in Mississippi.
• Position involves 8-hour day shifts, Monday to Friday, 8:00 AM to 4:30 PM.
• No weekend requirements; holiday shifts rotated as needed.
• Case load: 15-20 patients/consults per day.
• Responsible for inpatient/acute care case management and discharge planning across all inpatient units.
• Must have experience in a large academic medical center or Level 1 trauma center.
• Will collaborate with Respiratory Therapy and Pharmacy for discharge planning as needed.
• Orientation period: 2 weeks, including general facility orientation and working alongside the case management team.

Requirements

• Licensure: Active Mississippi LMSW or LCSW license required.
• Experience: Minimum 2 years of acute care management experience in social work case management.
• Prior Travel Experience: Not specified or required.
• Certifications: BLS certification required.
• Technical Skills: EPIC charting experience required.
• Must have level 1/large medical center experience in inpatient/acute care case management and discharge planning.
• Must comply with a 50-mile radius rule for local candidates.

Additional Information

• Flu vaccine is required unless medically exempt with documented evidence.
• Fingerprint background check required; can be completed remotely or in person prior to assignment.
• Social Work team consists of 15 full-time and 17 part-time social workers.
• The department currently does not have a case management manager; reports directly to the Social Work Manager.
• Dress code: Any color scrubs or business professional attire.
• The facility has contracts with local facilities to assist with complex patient placements, focusing on uninsured or underinsured patients.

This role is ideal for social workers with strong acute care and discharge planning experience looking to work in a high-acuity, trauma center environment.
