Registered Nurse (RN) - Long Term Acute Care Hospital (LTACH)

Job Overview

• Maintain delivery of quality nursing care by following the nursing care plan.
• Supervise Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees.
• Participate actively in patient rehabilitation programs on assigned units.
• Perform nursing assessments within 8 hours of patient admission to develop individualized care plans.
• Initiate discharge planning and patient teaching from day of admission.
• Communicate patient status changes to care team to ensure continuity and safety.
• Provide direction, training, and supervision to ancillary nursing personnel.
• Assume charge nurse responsibilities after orientation, as requested.
• Maintain personal and environmental safety according to hospital policies.
• Contribute positively to unit functioning and team building.
• Monitor supplies and equipment needs, ensuring cost-effective care.
• Pursue ongoing professional development and continuing education.

Key Responsibilities

• Conduct timely and accurate documentation of assessments, interventions, and patient responses.
• Collaborate with patients, families, and interdisciplinary teams to identify teaching needs and goals.
• Use a positive, encouraging approach to help patients achieve goals and quality outcomes.
• Delegate tasks appropriately and use problem-solving techniques in decision-making.
• Maintain communication with management regarding service needs and clinical training.

Required Skills and Experience

• BLS certification required prior to first shift.
• ACLS certification required prior to first shift (for CIRH positions).
• Minimum of 2 years bedside clinical experience.
• ICU experience or appropriate skills for LTACH setting.
• Previous experience with Select Medical preferred.
