Pharmacist Position Overview
Key Responsibilities
- Ensure patient and employee safety by adhering to safety policies and promoting a safe work environment.
- Maintain adequate drug supply, order medications timely, and manage emergency procurement.
- Complete annual inventory and manage removal of outdated medications.
- Comply with all Federal and State Pharmacy laws, rules, and regulations.
- Prepare, sterilize, label, and dispense medications accurately, including parenteral products.
- Maintain strict control and documentation of all dispensed drugs, including controlled substances.
- Conduct monthly inspections of pharmacy and patient care areas where drugs are stored or administered.
- Participate in development and updating of hospital formulary and provide it to professional staff.
- Provide education and training for pharmacy staff and other healthcare professionals.
- Counsel and instruct patients on medication use post-discharge, including providing written materials if necessary.
- Maintain confidentiality of patient and medical staff information.
- Participate in quality assurance programs related to drug utilization and effectiveness.
- Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness.
- Support antimicrobial stewardship and pharmacokinetic dosing management.
- Participate in Pharmacy and Therapeutics Committee meetings and implement decisions hospital-wide.
- Promote continuous quality improvement and support accreditation efforts.
Required Skills and Qualifications
- Successful completion of all educational requirements for state licensure.
- Current State Pharmacist licensure.
- BLS certification if required by the state.
- EPIC electronic health record experience.
- Minimum of two (2) years recent hospital pharmacy experience.
- Strong communication skills for medication reconciliation and discharge counseling.
- Clinical knowledge in:
- Antimicrobial Stewardship (interpretation of culture results, recommendations for UTI, pneumonia, SSTI, etc.)
- Pharmacokinetic dosing management and monitoring.
- Anticoagulation dosing management and monitoring.
Contract Length
- 13 weeks
This role requires a pharmacist who is detail-oriented, safety-conscious, and experienced in hospital pharmacy operations with strong clinical and communication skills.
