Position: Physical Therapist

Job Description

  • Evaluate assigned patients and develop, implement, and modify treatment plans based on patient needs and physician direction.
  • Delegate and direct discipline-specific therapy services for assigned patients.
  • Design functional treatment programs to support effective return to community activities.
  • Educate patients, families, students, and coworkers.
  • Perform professional duties related to patient care, therapy service development, and program delivery.

  • Collaborate with interdisciplinary teams to address discharge barriers, including equipment trials, patient/family training, and planning for continuing services.
  • Manage caseload flexibly, including coverage and communication of care plans.
  • Document medical records clearly, accurately, and timely, including daily notes, goal writing, quality assessments, letters of medical necessity, discharge documentation, and charge entry.
  • Ensure care is provided under valid physician orders and insurance approvals.
  • Participate in student programs by providing supervision, learning objectives, and evaluations.
  • Develop and pursue individualized professional development plans.
  • Engage in patient and professional advocacy activities.
  • Adhere to state practice acts, maintain licensure, complete continuing education, and uphold ethical standards.
  • Support organizational performance improvement initiatives.

Requirements
