Case Manager - Travel Assignment

Contract Length: 13 weeks (approximately 3 months)
Schedule: Monday - Friday


Job Overview

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, financial management, and completing medical record documentation. This role emphasizes patient and employee safety, collaboration with interdisciplinary teams, and ensuring compliance with payor and regulatory requirements.


Key Responsibilities

  • Identify patient problems affecting treatment plan participation and assist patients/families in utilizing hospital resources.
  • Collaborate with interdisciplinary teams, including physicians, to manage clinical plans of care and facilitate safe, timely discharges.
  • Conduct utilization reviews and ensure medical necessity criteria are met for admissions and continued stays.
  • Maintain up-to-date community resource systems and refer patients/families to appropriate outside agencies.
  • Ensure clinical documentation aligns with payor guidelines for reimbursement.
  • Participate in team meetings and contribute to facility-wide operational improvements.
  • Manage payor/referral satisfaction and assure timely insurance certifications.
  • Document plans of care and progress notes according to policy and state regulations.
  • Perform other duties as assigned.

Required Qualifications

  • Current licensure in a clinical discipline per state guidelines (RN, RT, or Social Worker preferred).
  • Working knowledge of the insurance industry and government reimbursement.
  • Minimum 2 years of experience in Case Management and Discharge Planning in an Acute Care setting.

Safety and Compliance

  • Adhere to safety policies and procedures, including safe patient handling and infection control.
  • Promote a culture of safety for patients and employees.
  • Follow facility guidelines on personal appearance, attendance, and punctuality.

Additional Notes

  • Strong communication skills to interact effectively with patients, families, healthcare teams, and payors.
  • Ability to identify barriers and develop solutions to support patient care and discharge planning.
  • Experience with utilization review and payor requirements is essential.
