Job Opportunity: Neuro Rehab Coach Supervisor (PRN Pool)

Job Description

• Supervise staff on assigned shift, providing guidance and communicating staff concerns to leadership.
• Oversee Neuro Rehab Coaches (NRC) in patient plan of care, collaborating with transitional team members.
• Assist patients with rehabilitation activities to promote education and engagement.
• Help patients achieve goals in various environments including home, therapy, and community.
• Coordinate staffing levels according to guidelines and patient needs.
• Assist patients with meal planning, food preparation, grocery shopping, and manage inventory.
• Perform suite management and housekeeping tasks as assigned.
• Coordinate and provide patient transportation, ensuring compliance with vehicle policies.
• Attend trainings and meetings as required.
• Complete and supervise documentation according to Neuro Transitional Rehab policies.
• Perform nurse delegation tasks including vital signs, gastric tube management, diabetic management, oxygen administration, medication crushing, catheter care, bowel and bladder program management, and trach care.
• Responsible for medication administration for up to 12 patients.

Requirements

• Current State LPN License: Required
• Current State Driver’s License in good standing: Required
• Current BLS Certification: Required
• Minimum 2 years of bedside experience in a residential care facility or rehabilitation environment: Required

Contract Length

• 13 weeks (approximately 3 months)
