Job Opportunity: Case Manager

Job Summary

The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, financial management, and completing medical record documentation. This role involves collaboration with interdisciplinary teams to ensure safe and timely patient discharges and adherence to payor requirements.

Key Responsibilities

  • Promote patient and employee safety, including safe patient handling and infection control.
  • Identify patient problems affecting treatment participation and assist patients/families in utilizing hospital resources.
  • Collaborate with physicians and clinical teams to manage clinical plans of care and discharge planning.
  • Conduct utilization reviews and ensure compliance with medical necessity criteria.
  • Maintain up-to-date community resource information and assist patients/families in accessing services.
  • Ensure clinical documentation meets payor guidelines for reimbursement.
  • Manage payor/referral satisfaction and insurance certifications.
  • Participate in team meetings and facility-wide improvement initiatives.
  • Document plans of care and progress notes according to policy and regulations.
  • Perform other duties as assigned.

Required Qualifications

  • Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred).
  • Associate’s Degree in a healthcare field required; Bachelor’s Degree preferred.
  • Working knowledge of the insurance industry and government reimbursement.
  • Minimum of 2 years of experience in Case Management and Discharge Planning in a Rehab Care setting.

  • 13 weeks
