Job Details

• Contract Length: 13 weeks
• Work Schedule:

  • Weekdays: 8:00 AM – 4:30 PM (Monday to Friday)
  • Weekends: 8:00 AM – 12:00 PM
• Employment Type: Contract

Required Qualifications

• Current State Pharmacist Licensure
• BLS Certification (if required by state)
• EPIC experience
• Minimum 2 years of recent hospital pharmacy experience
• Successful completion of all educational requirements for state licensure

Job Responsibilities

• Maintain adequate drug supply and manage procurement, including emergency orders
• Comply with all federal and state pharmacy laws, rules, and regulations
• Oversee monthly removal of outdated medications and document recalled drugs
• Prepare, sterilize, label, and dispense medications, including parenteral products
• Ensure competency assessments for staff preparing/administering large volumes of parenterals
• Maintain strict control and accountability of all drugs dispensed
• Record and document all dispensed drugs and maintain pharmacy transaction records
• Conduct monthly inspections of patient care units and pharmacy areas
• Manage controlled substances and maintain required documentation
• Replace drugs removed from emergency carts and floor stock
• Participate in development and updating of hospital formulary and provide it to staff
• Provide education and training for pharmacy staff and other healthcare professionals
• Counsel and instruct patients on discharge medications, providing written materials as needed
• Maintain confidentiality of patient and medical staff information
• Participate in quality assurance programs related to drug utilization and effectiveness
• Attend Pharmacy and Therapeutics Committee meetings and implement decisions
• Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness
• Provide clinical consultation and support antimicrobial stewardship and pharmacokinetic dosing management
• Promote patient and employee safety, adhering to safety policies and procedures
• Support accreditation efforts and continuous quality improvement initiatives
• Serve as a role model and resource for staff, fostering positive inter/intra-departmental relationships

Clinical Knowledge & Skills

• Antimicrobial Stewardship: Interpretation of culture results and recommendations for infections such as UTI, pneumonia, SSTI
• Pharmacokinetic Dosing Management and Monitoring
• Anticoagulation Dosing Management and Monitoring
• Medication reconciliation for discharge medication lists
• Effective communication for gathering home medication lists and discharge counseling

Safety & Compliance Focus

• Emphasis on patient and employee safety, including safe patient handling, infectious disease control, and workplace hazard awareness
• Adherence to facility guidelines on personal appearance, attendance, and punctuality

This position requires a pharmacist with strong clinical knowledge, hospital experience, and the ability to work collaboratively in a safety-focused environment.
