Job Opportunity: Pharmacist (PRN)

Contract Length

• 13 weeks

Required Experience

• Minimum 2 years of recent hospital experience
• Higher years of experience not specified elsewhere, so 2 years is the requirement

Licensure & Certifications

• Current state licensure required
• BLS certification if required by the state

Key Responsibilities

• Maintain adequate drug supply and manage procurement, including emergency orders
• Comply with all federal and state pharmacy laws, rules, and regulations
• Prepare, sterilize, label, and dispense medications accurately, including parenteral medications
• Ensure competency assessments for staff preparing/administering large volumes of parenterals
• Maintain strict control and accountability for all drugs dispensed, including controlled substances
• Conduct monthly inspections of pharmacy and patient care areas where drugs are stored or administered
• Participate in development and updating of hospital formulary and provide it to professional staff
• Document all dispensed drugs and maintain pharmacy records as required by law
• Maintain cleanliness and order of the pharmacy, including temperature checks
• Participate in new employee training, orientation, and ongoing education programs
• Provide medication education and discharge counseling to patients and families
• Maintain confidentiality of patient and medical staff information
• Participate in quality assurance programs related to drug utilization and effectiveness
• Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness
• Provide clinical consultation and support antimicrobial stewardship and pharmacokinetic dosing management
• Promote patient and employee safety, adhering to safety policies and procedures
• Support accreditation efforts and continuous quality improvement initiatives
• Foster positive inter/intra-departmental relationships and serve as a role model for staff

Required Skills & Knowledge

• EPIC experience required
• Medication reconciliation for discharge medication lists
• Strong communication skills for gathering home medication lists and discharge counseling
• Clinical knowledge in:

  • Antimicrobial stewardship (interpretation of culture results, knowledge of UTI, pneumonia, SSTI)
  • Pharmacokinetic dosing management and monitoring
  • Anticoagulation dosing management and monitoring
