Job Opportunity: Case Manager

Job Summary

The Clinical Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, financial management, and completing medical record documentation. This role involves collaboration with interdisciplinary teams to ensure safe and timely patient discharges and effective care plans.

Key Responsibilities

• Promote patient and employee safety, including safe patient handling and infection control.
• Identify patient problems affecting treatment participation and assist patients/families in overcoming barriers.
• Collaborate with physicians and clinical teams to manage clinical plans of care.
• Facilitate patient and family participation in care decisions and discharge planning.
• Conduct utilization reviews and ensure compliance with payor requirements.
• Maintain up-to-date community resource knowledge and assist patients/families in accessing services.
• Manage insurance certifications and billing-related documentation to minimize financial risk.
• Participate in team meetings and contribute to facility-wide operational improvements.
• Document plans of care and progress notes according to policy and regulations.
• Perform other duties as assigned.

Contract Length

• 13 weeks

Required Qualifications

• Current licensure as a Registered Nurse (RN) appropriate for the Case Manager role.
• Associate’s Degree in a healthcare field required; Bachelor’s Degree preferred.
• Minimum 2 years of experience in Case Management and Discharge Planning in an Acute Care or Rehabilitation setting.
• Working knowledge of the insurance industry and government reimbursement processes.
