Job Description

• Deliver superior quality pharmaceutical services while adhering to core values such as respect, teamwork, and resourcefulness.
• Prioritize patient and employee safety by following safety protocols including safe patient handling, infectious disease precautions, and workplace hazard reporting.
• Maintain adequate drug supply and manage procurement processes, including emergency medication orders.
• Complete annual inventory and manage removal of outdated or recalled medications.
• Prepare, sterilize, label, and dispense medications accurately, including parenteral and non-parenteral products.
• Ensure strict control and documentation of all dispensed drugs, including controlled substances.
• Conduct monthly inspections of pharmacy and patient care areas where medications are stored or administered.
• Participate in development and updating of hospital formulary and provide it to professional staff.
• Provide education and training to pharmacy staff and other healthcare professionals.
• Counsel and instruct patients on medication use upon discharge, including providing written materials as needed.
• Maintain confidentiality of patient and medical staff information.
• Participate in quality assurance programs related to drug utilization and effectiveness.
• Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness; report adverse drug reactions.
• Support antimicrobial stewardship and pharmacokinetic dosing management.
• Utilize EPIC system for medication reconciliation and billing functions.
• Participate in Pharmacy and Therapeutics Committee meetings and implement decisions hospital-wide.
• Exhibit professional workplace behaviors and support continuous quality improvement.
• Provide on-call availability as needed.

Required Qualifications

• Valid Hospital Pharmacist license in Ohio.
• Minimum of 2 years hospital pharmacy experience, including experience within the last year.
• Experience with EPIC system.

Additional Information

• Must be able to work under general supervision.
• On-call availability required.
• Strong communication skills for gathering medication histories and discharge counseling.
• Clinical knowledge in antimicrobial stewardship (including interpretation of culture results and common infections) and anticoagulation dosing management/monitoring.
